@ashcroftethics Evidence based medicine: a movement in crisis? A paper that nearly didn’t get published at all (Mike Rawlins wrote a scathing review and recommended rejection) but it went on to be BMJ’s highest-cited paper of 2014. https://t.co/h1oVPMPRs
is there a problem with over reliance on evidence? does it under value tacit knowledge developed with clinical experience? https://t.co/dD3yjgxlS0
Goldacre is iften quotes by ‘what works’ advocates but for a more nuanced view of evidence based medicine see Greenhalgh (2014) which focuses on ethics, judgement, dialogue with patients and community practice. https://t.co/Q8L2bfgAsq

Reading this 5 years on, it sounds like my living mantra. Box 1 nailed it for me. Context, complexity, tacit (even anecdotal) knowledge. People are not ‘average’. Real patients seldom fit the ‘not lost to follow up’ profile that features in most EBM data.

Too much evidence, marginal gains and poor fit for #multimorbidity. Potential solutions: tailoring, more judgement, less rules, shared decision - renaissance and raising the bar? 5 yrs later have we met much progress @trishgreenhalgh ?
